If the difference between a good connection and a bad one is only the slight offset of the adapter connection, It says to me that the internal connections of the adapter have an insufficient contact surface and results in a hot connection. I have noticed that when I get the error message, I can touch the wall plug and the connector and the connector is much warmer than the wall plug (40 amp circuit with Hubbel outlet). What I do is to spray electrical contact cleaner on both the adapter plug and the mobile connector contact points.
